We appreciate that you have chosen our practice and hope you will find your experience here productive and enjoyable. Below, you'll find the forms & insurance information we will need from you prior to your initial appointment, as well as our location & contact information. Please submit all forms and insurance information at least 48 hours in advance of your appointment.
We are required to provide you with certain forms which let you know how our practice operates and what your rights are as a patient. Please take the time to closely read and fill out the necessary forms in detail so your clinician can get to know a bit about you & your goals prior to you initial appointment:
For All Clients
- Personal Information Form (18+) or
- Personal Information Form for Minor Children (17 & under)
- Medications Form
- Confidentiality Form
- Acknowledgment of Privacy Practices
- Cancellation Policy
- Authorization Form
For Group Clients Only
If you haven't already done so, please submit your insurance information to our Billing Manager, Carolyn. She can be reached by email at [email protected] or by phone at (908) 431-5254 extension 2. Carolyn will need the following information from you:
- Full name
- Date of birth
- Insurance carrier
- Group/ID #s OR a picture (front and back) of your insurance card
You will then receive an email confirming your insurance benefits and co-payment amount (if applicable). Co-payments are due at the time of service. We accept all major credit cards, cash, or checks (made payable to Princeton Group Support Center). In order to hold your appointment, please be sure to submit your insurance information at least 48 hours in advance of your initial visit.
We are located at 88 Orchard Rd, Skillman, NJ 08558, in the Farmhouse at the Orchard Hill Center. There is parking available on site. When you arrive, please have a seat in the waiting area. Your therapist will come out to greet you shortly.
At PGSC, we believe that healthcare is a two-way street. This means that in addition to providing you with excellent healthcare, we are also aware that you are a "consumer with choices" and that our success depends on your having an outstanding experience here.
